Per service definition dynamically configurable form (Simple Form)

Some fields, like "name" should probably be hard-coded as they are mandatory for evey service. There will be other upcoming fields that will be mandatory for every service.

Improve detection of mandatory fields

For now we change the field in the backend: https://vshnticket.atlassian.net/browse/APPCAT-1188 This way we "buy us some time" in the portal.
