feat(staff): link into Grafana from the control plane views

Staff had to build Grafana URLs by hand, starting with a cluster ID the
portal did not store at all.

ControlPlane gains a manual `cluster_id` field, and `SERVALA_GRAFANA_URL`
points at the internal Grafana. `core/grafana.py` builds the three deep
links (cluster, node, namespace) and returns None whenever either is
missing, so a plane without an ID simply shows no links.

Only data-selection query parameters are kept (datasource, node exporter
job, all creators). Time range, refresh and timezone are left to Grafana.

Links show on the control plane list and detail, on every node row, and on
the staff instance detail, which gains an "Instance namespace" row. The
list tooltip on the plane name now carries the cluster ID instead of the
K8s API server.
